1. ISSUE OF VALUE CERTIFICATES

* Application filled by the applicant.

* The certificate of grama niladhari after investigation.

* Deep copies certified by the grama niladhari.

* Folds for the deeds.

* Changes will be made at 1.26 of the value of worth.

 

2. ISSUING REVENUE CERTIFICATE

* Application according to the form provided by the grama niladhari.

* Documents in support of sources of income.(In case of vehicles a document of registration and in case of business, business registration certificate)

* If the application is a certificate of annual in come a receipt in support of payment of 1.26 percent.

* In case of monthly income certificate a receipt for the amount payable monthly.

* Certificate of income should be issued in 15 minutes of handing over of the receipt.

 

3. ASSESSMENT REPORTS

* Applicant should be a government employer.

* A letter should be forwarded by the applicant through the head of the institute applying a assessment report to issue free from the institute. Deed of the land should be forwarded herewith too.

* You should met the grama niladhari of relevant grama niladhari division after this documents handed over to the divisional secretary.

* The assessment report form filled by the grama niladhari should be forwarded to the divisional secretary.

* On approval by the divisional by the divisional secretary, subject clerk will prepare the assessment report and it will be issued to the applicant after it is signed by the divisional secretary.
